This California Privacy Rights notice supplements the Privacy Policy for Constellation Health and applies to California residents under the California Consumer Privacy Act, as amended by the California Privacy Rights Act.

Depending on your interactions with Constellation Health, personal information may include identifiers and contact details, account information, order and transaction records, communications with support, and website or device activity. We collect information directly from you and through the services described in our Privacy Policy.
Our storefront offers research-use products. This notice describes customer accounts, orders, and website interactions.

Submit a verifiable request through support@constellationhealth.app. We may need information to verify your identity and authority to act, including when an authorized agent submits a request. We respond within applicable time limits and will notify you when an extension is permitted and needed.
Some information may be retained where necessary to complete transactions, provide requested services, protect security, or comply with legal obligations.
